Back-to-School Resource Fair

The Nevada Office of Minority Health and Equity (NOMHE) is collaborating with Fountain of Hope to host the Back-to-School Resource Fair, where students can receive sports physicals and other health services. Hope Clinic will be on-site providing immunizations.

The event is located at Helen Cannon Middle School at 5850 Euclid St., Las Vegas, Nevada, from 10 AM to 1 PM on July 25.

All services are first-come, first-served. To expedite sports physicals, we request the public to scan the QR code on the attached flyer to download the forms and bring them completed to the event for faster processing.
